Adding your first patient

You need at least one patient before you can record a session. Adding one takes about 30 seconds.

Steps

  1. Click + New at the top of the sidebar (or press ⌘N)
  2. Fill in the Full name — required
  3. Optionally add date of birth, pronouns, and intake notes
  4. Click Create patient

The patient appears in the sidebar list. Click their name to open their detail page.

What each field does

  • Full name — how the patient appears in your sidebar and in their detail page header. Required.
  • Date of birth — used for age display on the patient detail page. Doesn't appear in notes.
  • Pronouns — when set, Confidant will use these pronouns in generated notes ("he reported," "they expressed"). If left blank, the AI uses third-person clinical voice ("client reported").
  • Intake notes — free-form text for referral source, presenting concerns, or anything else you want at hand when you open the patient. Not sent to the AI when generating notes — that's what the Case Overview field is for, which builds itself from your sessions over time.

After you've added a patient

The patient detail page lets you customize a couple of things just for this patient:

  • Therapy modality — override the default modality just for this patient (e.g. you mostly do CBT but this patient is in DBT)
  • Note format — override SOAP/DAP for this patient (Default uses your global setting)
  • Case overview — auto-generated summary that updates as you finalize notes

Privacy mode

The eye icon in the sidebar header (next to + New) toggles Privacy Mode — patient names show as initials instead of full names. Useful for screen sharing or working in public. Toggle with ⌘⇧P. The setting persists between launches.
