Per-patient customization

Written by Jason

Most settings apply to every client and live in Settings. Some can be set for one client only, and they override your global settings for that client.

Where to find them

Open the client's page. Per-client settings are in two places:

  • Client settings, the button at the top right of the page (Patient settings if you've switched Confidant to say "patients"). It has three tabs: Profile, Notes and Recording.

  • The Case overview card on the client's page, for diagnoses and treatment goals.

If a client has no pronouns set, the line under their name shows + Add pronouns. Click it to go straight to the Profile tab.

What you can set

Client settings › Profile

  • Pronouns: He / Him, She / Her or They / Them. Notes use them to refer to your client ("she reported," "they expressed").

  • Private notes: for you only. They never go into a note.

Client settings › Notes

  • Modality: Your defaults are pre-selected. CBT, DBT, ACT, Psychodynamic, Person-Centered, Integrative, EMDR, IFS, ERP, CPT, EFT (Couples), Gottman (Couples) or Other can be selected. Turn on as many as apply. Notes are written in the clinical voice of the modalities you pick.

  • Note format: Default, SOAP, DAP, BIRP, GIRP, PIRP or SIRP. Default follows your global format and shows which one that is (for example "Default · SOAP"). Pick a format to always use it for this client.

  • Mental status exam: Not decided, On or Off. Not decided asks at the first session. On offers the exam every session, and it stays optional. Off hides it for this client. See Recording a mental status exam.

  • Note prompt: text that fills in Instructions for this session each time you open a session with this client. You can still change it for any one session. Clear the box to remove it.

Client settings › Recording

  • Usually: Ask each time, In person or Telehealth. New sessions for this client start set to your choice, and you can change it before recording. Ask each time picks based on whether a video call app is running.

  • Auto-stop recording: Default, Off, 30, 45, 50, 60, 75 or 90 min, or 2 hr. Two minutes before it stops, Confidant asks whether to keep going. Paused time doesn't count. Default follows Settings › Sessions › Recording.

Case overview card

  • Diagnoses: click Add (or Edit once there are some) and type a code or a name, like F41.1 or anxiety. Click a result to add it. Notes mention diagnoses by name. Codes stay on the client's list and don't appear in the note.

  • Treatment goals: click Add (or Edit) and write one goal per line, in your own words. Then click Save. Notes mention a goal only when the session worked on it, usually in the Assessment section.

Settings that apply to every client

These can only be set in Settings, for all clients:

  • Detail level (brief, standard or detailed)

  • Direct quotes (none, some or many)

  • Custom instructions. For one client only, use their Note prompt.

  • Your own pronouns

Saving and when changes apply

Client settings save as you go, so there's no Save button. Click Done to close. Treatment goals are the one exception: click Save when you're finished.

Changes apply to new notes only. Notes you've already written stay as they are. To update one, regenerate it. After a format change, Confidant asks whether to switch that note to the new format or keep its own.
