Customizing how your notes sound

Written by Jason

The AI writes in your voice as much as it can — but you tell it what your voice is. Five settings shape every generated note.

1. Therapy Modality

The clinical framework you practice. Sets the vocabulary the AI uses for interventions and assessments.

  • Settings › Profile › Modalities — your global default

  • Per-patient override on the patient's page: click Client Settings on the top of the client page

You can pick more than one — useful if you blend (e.g. CBT + DBT, or person-centered + EMDR). The AI uses language consistent with whichever modality is selected for the patient.

Available modalities: CBT, DBT, ACT, psychodynamic, person-centered, integrative, EMDR, IFS (Internal Family Systems), ERP (Exposure and Response Prevention), CPT (Cognitive Processing Therapy), EFT and Gottman for couples, and Other.

2. Note Format

SOAP, DAP, BIRP, GIRP, PIRP or SIRP. See Note formats for what each one writes and which to pick. The picker lists the sections each format produces.

  • Settings › Notes › Note style › Format — your global default

  • Per-patient override on the patient's page: click Note format in the strip under their name

3. Note Detail Level (density)

How long each section should be:

  • Concise — the shortest, tightest notes Confidant produces, roughly half the length of Standard

  • Standard — default; balanced depth, covers the session thoroughly without extra elaboration

  • Comprehensive — full clinical detail, 5–7 sentences per section; captures nuance and quotes

Set in Settings › Notes › Note style › Detail level. There's no per-patient override — pick the density that matches your documentation style overall.

4. Direct Quotes

How often direct quotes from the client appear in the note:

  • No quotes — no quotes; everything is paraphrased in clinical voice

  • Some quotes — default; quotes appear where they meaningfully ground an observation

  • Many quotes — quotes used liberally to capture the client's voice

Set in Settings › Notes › Note style › Direct quotes.

5. Custom Instructions

Free-form text prepended to every note generation. The AI receives this as instructions to follow precisely. Use it for things you want in every note that aren't covered by the structured settings.

Examples:

  • "Always note attachment style observations in the assessment."

  • "Use British English spelling."

  • "Include a one-line safety scan even when no risk is present."

  • "Refer to me as 'this clinician' rather than 'therapist' in the note."

Set in Settings › Notes › Note style › Your instructions; it saves when you click out of the box. This applies to every note across every patient. For one patient, use the Note prompt box on their page instead, and for one session, the Session notes for this generation box above the Generate button (see Per-patient customization).

After changing any setting, regenerate the note for it to take effect. Existing notes don't auto-update, and regenerating keeps the sections you edited (see Regenerating a note).
